Where are you located?

This is a 100% online practice dedicated to patients living and studying all across Iowa. From Sioux City to Cedar Rapids, patients easily attend appointments using their phone, tablet, or laptop.

Before your first visit, please complete the intake forms, which can be sent to your email. Forms must be completed no later than 72 hours prior to your appointment. I thoroughly review all of your information and prepare with patient-specific recommendations beforehand. Thank you for giving me time to do this properly.

For your comfort and privacy, please plan to attend your appointment from a quiet, private space where you will not be interrupted, you can sit in your car, just not driving please.

Do you prescribe controlled substances?

Yes, if that is the right option for your care. I’ll complete an evaluation to better understand your or your child’s needs and support the treatment that fits you best.

What ages do you treat?

I have experience treating kids, teens, adults, and seniors. Yes, older adults can suffer from ADHD.

Do you accept insurance?

Wellness Pattern Studio PLLC accepts Wellmark BCBS; Alliance Select and Classic Blue PPO and cash pay.

Also know that many insurance plans can reimburse up to 100% of my services. Why see an out-of-network provider? Appointments are tailored to your needs, and additional time spent outside of sessions is devoted to improving your care. Your appointments will never be rushed and times are flexible, be seen when it is best for you. You can still use your insurance for medications.

To celebrate my launch, I’m offering reduced rates for patients who begin care before September 8th.

Founding Patient Rates:
New Patient Consultation: $150
Follow-Up visits: $85

These rates are locked in for the life of your care as long as you remain an active patient.

Standard rates beginning September 8th:
New Patient Consultation: $225
Follow-Up visits: $125

Certain services may be eligible for FSA or HSA reimbursement, and the Superbill includes the date of service, CPT codes, diagnosis codes, and fees to assist in submitting a claim. However, please note, that reimbursement from your insurance, FSA, or HSA is determined by your plan and is not guaranteed.

You will be provided with a Superbill or itemized receipt for services such as psychiatric evaluation, medication management, and psychotherapy upon request. If you have out-of-network insurance benefits, you may submit this Superbill to your insurance company for possible reimbursement.

Payment (or co-payment if you have one) is due on the day of your appointment and will be charged to the credit card on file.
